Todd Gamblin 9c37a4c948 meta: move CODE_OF_CONDUCT.md and CONTRIBUTING.md to .github 
- Trying to reduce the number of top-level files in Spack.

- CODE_OF_CONDUCT.md can be referenced from the README.

- CONTRIBUTING.md is only used to display a message on pull requests, and
  it still does that when in .github.

Contributing to Spack

All contributions to Spack must be made under both the Apache License, Version 2.0 (Apache-2.0) and the MIT license (MIT).

Before contributing to Spack, you should read the Contribution Guide, which is maintained as part of Spack's documentation.
